Create a HealthEI QR code for your lockscreen and personalize it with photos.


The HealthEI App is a simple way for Essential Health Information to be accessible when there is an emergency situation, where someone is incapacitated and unable to communicate, or for more general uses such as child security when attending school or playing sports. You create and store a unique personal QR code on the lock screen of your phone and can personalize the lockscreen with a collage of photos. It can be read by any standard QR code reader and contains only the health information you want to share in the event of an emergency. Parents can share the HealthEI code via email with school administrators, teachers, sports coaches, and other guardians to ensure that their child’s essential health information is available when needed and the parent can be contacted without delay.The HealthEI QR Code contains essential emergency information but leaves it up to the user as to what information is shared. At a minimum, the name and contact details of the phone owner, and the emergency contact details are required to be populated. The user determines their own level of privacy by sharing information considered to be essential. Users may not choose to include extremely sensitive information but more complete information can be obtained by contacting the emergency medical contact for that person.The app requires the following fields to be populated*Name and contact details of phone owner*Person to contact in an emergency- Usually a family member or close friend but could be the primary medical contact.The following fields are not required but are at the option of the user. These are free text fields.Medical Conditions- Include serious medical conditions here. Examples include diabetes, hypertension, or the presence of a medical device such as a pacemaker.Medications- List of medications or supplements to avoid contra-indications in any treatment.Allergies- Any environmental, food, or medication allergies. Examples include allergies to penicillin or nuts.Notes- Any general or more specific information that can assist the person trying to help.The QR Code can also be printed on labels for bags, photo IDs, and articles of clothing.Bug fixing and UI improvements
